Who the Social Support Program
is for:
• Older socially isolated men and women and
• Living in low cost accommodation
What is provided:
• Social group outings
• Individual support and assistance
What is the purpose:
• To reduce social isolation by providing the participants
with an
opportunity to mix in a social setting
• Participants are encouraged to develop ongoing
interests and
contacts
Who is eligible:
• People over 50
• On low income
• Living in low cost housing
• Homeless or at risk of homelessness
• Eligible for HACC services
• Socially isolated
• Living in Maribyrnong or Moonee Valley
What it involves:
• Small group outings to parks, gardens, museums,
films etc.
• Picnic afternoon tea
• Participants picked up from and returned to their
home
• The program is free
How to get into the program:
• Contact the program worker/s
• Fill in a referral form with the worker/s to determine
eligibility
• Assessment in person with the support worker/s
• Join the program
